Prediabetes is a serious health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. The CDC-led National Diabetes Prevention Program helps people make lasting lifestyle changes to prevent or delay type 2 diabetes. If you qualify for the Program you will have the opportunity to work with a trained coach, learn about healthy food and exercise, and manage stress and other factors.
Shore Physicians Group is proud to offer a CDC-curated Diabetes Prevention Program at no cost, which meets regularly to help people at risk for type 2 diabetes delay or even prevent the disease by making the necessary lifestyle changes to stay healthy. Those interested in this free program should take the Prediabetes Test above to determine their eligibility. Once you have submitted your results, eligible participants will be contacted by Shore Physicians Group to enroll in the program. In the program, you will not only learn how to prevent Type 2 Diabetes, but build relationships and skills that will last a lifetime.
Now in its third year at Shore Physicians Group, the Diabetes Prevention Program provides you with the tools you need to be successful, including a digital scale, portion control tools, exercise support, meal planning guidelines and more. Sessions are held once each week for the first six months, bi-weekly for the next three months, and once a week for the final three months.
Sessions will be held virtually via Zoom.
For upcoming sessions and more information, contact Angela Insana at diabetesprevention@shorephysiciansgroup.com or call 609-365-6200 ext. 6018.
